A hybrid instrument can achieve two-dimensional viewing and analysis of the specimen during laparoscopic pelvic lymphadenectomy (LPL). The Laparoscopic Instrument for Lymph Node Dissection (LIND) is a prototype hybrid device that provides the surgeon with a stable and dexterous surgical instrument in place of the laparoscopic grasper to achieve two-dimensional viewing of the specimen during the extended LPL procedure. The LIND was used for 91 consecutive patients undergoing LPL for stage II or III cervical cancer from 2014 to 2016. This report describes the surgical technique, postoperative course, and oncologic efficacy of this technique during a 5-year period at two different academic institutions. LPL was performed with the LIND for 91 consecutive patients between March 2014 and March 2016 at two institutions. In all cases, LPL was performed by the same surgeon using the initial technique described by Song et al. (Journal of Minimal Access Surgery 2015;19(5):306-310) modified by the development of a stable and dexterous laparoscopic instrument in place of the laparoscopic grasper. The LIND consists of a linear laparoscopic instrument that is connected to a movable arm that houses a magnetic endoscopic instrument. The arm is deployed in the peritoneal cavity through the laparoscopic port using a magnetic-based robotic arm. This allows the surgeon to remotely view the specimen on a two-dimensional monitor using the laparoscopic instrument while the specimen is inside the magnetic arm, which is fixed in place using a magnetic system. The specimen can be retrieved using a standard laparoscopic instrument. Blood loss and operative time were recorded. The number of dissected pelvic lymph nodes was recorded as the specimen was retrieved. All patients were followed up for a minimum of 2 years, and data on recurrence, second primary tumor, and survival were also recorded. The mean age of the patients was 43 years, and the mean body mass index was 20.6. Of the 91 patients, 87 had no evidence of recurrence at the time of follow-up.
